All sworn statement of affidavits must be properly notarized. Indicating therein the proof of identification of the party being sworn by the notary public. 2. All documents to be submitted must be properly authenticated. Documents executed outside of the Philippines must be authenticated by the official of the Philippine foreign service at the place of execution or nearest to it. Any document executed within the Philippines must be duly certified by the offices having official custody of the originals. 3. All required documents required in nos. 1 & 2 must be submitted and must be in order, otherwise your application or petition cannot be processed immediately. CHECKLIST OF REQUIREMENTS FOR THE CHANGE OF STATUS FROM TEMPORARY VISITOR TO NON QUOTA IMMIGRANT BY MARRIAGE (Under Section 13(a) of the PIA of 1940, as amended) 1. ( ) Letter of application 2. ( ) General application form duly accomplished and notarized (BI Form No. RBR 98-01) 3. ( ) 2 x 2 ID picture to be attached to the application form 4. ( ) Birth certificate of applicant / Filipino spouse in the absence of the BC, submit the following: [ ] a. Certification of the Local Civil Registrar or the National Statistics Office of its loss or non-availability [ ] b. any public document which will establish the Filipino citizenship of the spouse [ ] c. Affidavit of two (2) disinterested persons indicating nationality of parents of Filipino spouse, and his / her birth place and birth date of applicant 5. ( ) True Copy of the Marriage Contract of alien and Filipino spouse 6. ( ) Certificate of legal capacity to contract marriage issued by the Embassy of Consulate of the alien spouse or official family register with authentic interpretation as certified by the Philippine Embassy of Consulate 7. ( ) Joint affidavit of legal capacity to contract marriage stating any previous marriage(s) and the manner by which the marriage was dissolved 8. ( ) If alien spouse was previously married a true copy of the divorce decree dissolution of marriage or death certificate as the case may be duly authenticated 9. ( ) True copy of Birth Certificate of minor unmarried children under 21 years, if any 10. ( ) Joint affidavit of financial capacity and support executed by the applicant and its alien spouse with supporting documents showing capacity to support themselves 11. Medical Certificate from the Bureau of Quarantine that alien spouse and unmarried children are not afflicted with any contagious, dangerous and loathsome disease 12. ( ) NBI clearance 13. ( ) BI Intelligence Clearance 14. ( ) Copy of passport of alien spouse and children showing date of arrivals and validity of stay CERTIFICATION This is to certify that the documents submitted in support of the application / petition for __________________________________________________________ are complete and in order. ( ) Letter request from the petitioner - (Filipino spouse) 2. ( ) General application form (BI Form No. RBR 98-01) duly accomplished, signed by both petitioner and applicant and notarized. 3. ( ) 2 x 2 picture to be attached to the General Application Form 4. ( ) Photocopies of the following: a. Passport with stamped of the visa b. Alien certificate of Registration (ACR) c. Immigrant Certificate of Registration (ICR) 5. ( ) Joint affidavit of the petitioner and the applicant stating that; a. they are husband and wife and are living together b. they are financially capable of supporting each other Important: Supporting documents to prove financial capacity such as Proofs of joint bank deposits or bank certification, Income Tax Return, Land titles, contract of employment, pensions, etc. must be submitted. 6. ( ) Barangay certificate that petitioner and applicant are resident of the barangay and that they continue to live together as husband and wife. 7.
