H-1B TEMPORARY WORKER VISA CHECKLIST
Please obtain the following items in order to properly document your H-1
petition:
I. Information Regarding Employer:
1. Name, Address, Telephone Number, Fax Number
2. Address where alien employee will work if different from above
3. Brief description of business nature
4. Date business established
5. IRS Tax ID number (EIN)
6. Current number of employees (please specify how many full-time and how
many part-time employees)
7. Number of employees currently under H-1B status
8. Company Gross Annual Income
9. Company Net Annual Income
10. Name and title of person to sign the H-1B petition
11. Job title, proposed job duties, salary offer to the alien employee
12. Supply us with 10 blank company letterhead
II. Information Regarding Alien Employee:
Information Regarding Alien Employee:
(a) Full First, Middle and Last Name (including other names ever used)
(b) Present home address and telephone number in US
(c) Date of birth
(d) Place of birth (city, state, country)
(e) Permanent home address outside of US
(f) US Social Security Number (if any)
(g) Job title, brief job duties, salary offer to alien beneficiary by the
petitioning employer
Information Regarding Dependent (if any):
(h) Full First, Middle and Last Name (including other names ever used)
(i) Date of birth
(j) Place of birth (city, state, country)
(k) Permanent home address outside of US
(l) US Social Security Number (if any)
(m) Detail information if dependent has ever been employed in the U.S.
(n) Detail information if dependent has ever filed immigration or has any
immigration application filed on his/her behalf
Please provide the following documents for Alien Employee:
(o) Copy of Detailed Resume
(p) Copies of college and post-graduate degree diplomas, degree certificates,
school transcript or marksheet (must be accompanied with English Translation
if a foreign degree)
(q) Copy of foreign degree equivalency evaluation statement (if any)
(r) Copy of paycheck stubs for the past 2 months as evidence of continuing
employment with current employer if currently holding H-1/L-1 or other types
of employment visa
Please provide the following documents for Both Alien Employee and Dependent:
(s) Copy of passport main pages (including passport validity page, all the
visa pages)
(t) Copy of most recent I-94 card, front and back (please remove I-94 from
passport when making copies)
(u) Copies of documents showing evidence of beneficiary and dependent’s
prior and current status in US such as: (please provide all that apply to
alien beneficiary and dependents):
i. H-1 or H-4 approval notices (if H status)
ii. L-1 or L-2 approval notices (if L status)
iii. Form I-20 if F-1/M-1 status (if F or M status)
iv. EAD card
v. Form IAP-66 (if J-1 status)
vi. Any other USCIS approval notices
(v) Copy of Certified Marriage Certificate (if spouse applying for H-4) and
Minor Child(ren)’s birth certificate (if minor children applying for
H-4).Documents must be accompanied with English Translation.
(w) Information if alien beneficiary and/or dependents have ever been in
exclusion or deportation proceedings or have been arrested or convicted of
any crimes
(x) Information if alien beneficiary and/or dependents have any immigration
petitions filed on their behalf or have any immigration application pending
(y) Information if alien beneficiary has ever been denied H-1 classification
